About Yourself, LLC is a minority and women owned organization with a strong foundation of over two decades of combined experience in health care coordination, government programs, small business ownership, and service to diverse communities across Minnesota.
Our mission is to provide high-quality, culturally responsive, Consultation Services (CS) under the Community First Services and Supports (CFSS) program. Our design is guided by person-centered, self-directed principles to ensure services are accessible, equitable, and compliant.

Our Services
About Yourself provides consultative services by educating participants on the CFSS program, explaining the agency and budget models, helping our participants to choose the best model for them, assist them with establishing person-centered Service Delivery Plans (SDPs), working collaboratively with Interdisciplinary Care Teams (ICT) of our participants, helping connect participants with CFSS and/or Financial Management Services providers (FMS), as needed, and last but not least, helping participants to monitor and update SDPs.
